Enter the code given to activate the account

Follow steps to input registration data

Print off sports screening paperwork if needed

Upload sports screening paperwork signed and stamped by a physician

Finalize with electronic parent/student signatures
Multiple Sport Athletes:
If you are athletically cleared to participate in a Fall or Winter sport, you will not need to have another physical performed. However, you must select Track & Field as one of your sports on the athleticclearance.com portal.
